Taking notes with technical cameras

I wonder what do people do or not do to make notes when using a tech is camera and digital back ( or indeed a film back) ?

Shutter speed, ISO and GPS via Capture Pilot plus date and time is about all that shows up in the metadata. I'd like to record aperture, focus setting, degree of tilt or shift

My handwriting is awful and even worse when standing and holding a notebook in my left hand. I've tried recording in Evernote via my phone. I've got a small dictating machine but it feels strange talking to myself.

Has anyone got a good method ? Partly it takes discipline on my part I guess. Or do people just not bother ?
