Status update: I was not able to figure out where to send the lens for repair, so I contacted BH Photo to see if they have any suggestions.
Long story short, they graciously offered to replace my defective lens with a good one (not sure if new).
The exchange was very fast - I dropped my lens at FedEx on a Friday, and got the replacement in my hands by Tuesday (4 days)!
The replacement seems to work very well.
I am very impressed by BH Photo's support, and extremely grateful to them!
